The short list

You do not need a filing cabinet. You need the small set of documents that someone might genuinely ask for, grouped by why they matter.

  • Identity. Passport or birth certificate, and the NHS number if there is one.
  • Permission. Lasting power of attorney for health and welfare, and for finances, plus any advance decision.
  • Instruction. The current medication list, the care plan, and any written preferences about treatment or residence.
  • Practical. Repeat prescription, key contacts, and a note of where the originals are kept.

Where families lose them

  • Spread across four people. Each sibling has a different piece, and nobody has the whole set.
  • Behind a login only one person knows. The document exists and is unreachable.
  • In a filing cabinet three hours away. Perfectly organised, completely useless in an emergency.
  • Out of date. A power of attorney that was replaced, or a care plan from before the last admission.

A filing rule that survives real life

  1. Choose one shared place. Not two, and not one per household.
  2. Give every document a date in its name, so the current one is obvious.
  3. Delete or archive superseded versions the moment they are replaced.
  4. Tell everyone in the circle where the place is, in one sentence they can repeat.
  5. Keep the originals somewhere physical and known, and say so in the shared place.

The annual review

Once a year, and after any hospital stay, walk through the pack and check three things: is it still current, can everyone who needs it find it, and does the contact list still work. Fifteen minutes, once a year, removes most of the pain.

Keep this alongside the rest of the plan. If you are still building that, start with coordinating care for an ageing parent.

The best document system is the one a tired relative can use at 2am without asking anyone.
